When Your Inspiration Photo Isn’t Real: AI, Rhinoplasty and the New Consultation

Plastic Surgery

September 30, 2026 | 1 minute read

Patients have always brought inspiration photos to a rhinoplasty consultation. What’s new is what’s inside that folder: increasingly, the photos are AI-generated, or an AI-enhanced version of the patient’s own face, built to a standard no camera — or scalpel — was ever designed to match.

Why does that matter? An algorithm and a surgeon are solving two different problems — one arranges pixels, the other works with bone, cartilage and healing tissue. When those two get confused, a consultation can quietly shift from “here’s what I’m drawn to” into “here’s exactly what I expect.”

Dr. Jonathan Schwitzer was recently interviewed by the American Society of Plastic Surgeons on how AI-generated faces differ from real anatomy, why the nose is especially vulnerable to this kind of digital distortion, and how surgeons separate what a patient loves about an image from what it can actually promise.

Below, Dr. Schwitzer explains how he evaluates an AI-enhanced inspiration photo during a rhinoplasty consultation — and how patients can use these images productively without mistaking a digital ideal for a surgical plan.
